In-Unit Maintenance Agreement/Application: This Agreement is used to enter
into an agreement to allow the maintenance personnel of Skyline House to perform repairs
within a unit. This form must be completed prior to Skyline House performing any non-emergency
in unit maintenance for the owner. Only an owner can execute this agreement.

Emergency Contact Form: This form is used by Management to contact an owner
while they are away from their unit. The owner may also provide an alternate contact name
on the form.

Guest Parking Agreement Form: This form is used by the management office and
residents in the event a resident is displaced from their parking space.

Occupancy Form: This form lists the names of those residing in a unit. The
form also includes information on a resident’s employer and emergency contact information.

Occupancy Rules Extract: This document lists the rules for the maximum number
of residents in a unit and covers guests.

Parking Space Rental Agreement: This form is the rental agreement between the
owner of a parking space and the renter of the parking space.

Party Room Information Package: This package contains all the forms and
documentation needed to reserve a party room for an event. The package contains:

Party Room Worksheet: This is the form a resident need to complete to
rent a party room. Items covered are guard(s), number of chairs and tables needed,
number of guests attending and the date and time of the event. Prices can be found
in the Party Room Information Section of the package.

Party Room Information Section: This section of the package lists
the costs of renting a party room and other pertinent information.

Rules and Regulations Section: This section lists the rules and
regulations governing the use of the party room under Rule 23. The Section is taken
from the Skyline House Unit Owners’ Association Rules and Regulations.

Guest Parking Section: This section covers the cost for guests to
park in the Skyline office complex.

Security Guard Duties Section: This section covers security guard duties
to be performed before, during, and after the event.

The Guest List Section: This section provides a place to enter guest names.
Maximum of one hundred (100) guests per event. The list needs to be completed and returned
to the management office no less than seventy-two (72) business hours prior to an event.

Authorization Agreement for Preauthorized Payments Form: This form authorizes
Skyline House to make electronic withdrawals (Automated Clearing House or ACH withdrawals) from an
owner’s bank account. It also is used to authorize Skyline House to use the owner’s credit card
to make a payment. Owners have the option to pay the monthly assessment, or the entire balance
owed on the account each month.

Storage Rental Agreement Form: This form is used to authorize a resident to rent
a storage locker from Skyline House.

Vehicle Registration Form: This form allows a resident to register their
vehicle with Skyline House.
